Pre-Appointment Intake eForms
All pre-appointment intake eForms will be delivered to your smartphone via secure-text 2-days prior to your appointment. We require electronic completion of all intake forms—prior to your appointment—to make the check-in process a breeze for all patients.
If you opt-out of Klara, you will need to arrive to your appointment 30-minutes early to complete all intake paperwork.
